Georgia Katie Beckett is a mail/fax packet to the Centralized Katie Beckett Medicaid Team in Norcross (fax 678-248-7459), with document upload also available through the DCH Katie Beckett portal since April 15, 2026. The physician-signed DMA-6(A) is valid only 90 days from signature, so time the doctor visit and mail promptly.
New Mexico's waiver chain is paper end to end: register with the Pre-Service Intake Bureau by phone, mail, fax, or in person; DDSD mails the application packet; the completed packet and every later step travel by mail.
Indiana is portal-first: FSW/CIH applications go through the BDS Gateway (paper State Form 55068 remains a fallback), while Health & Wellness and PathWays start with a phone call to the Area Agency on Aging. Note: FSSA announced FSW/CIH reached capacity in December 2025, with no new slots expected until at least July 2026 — applying still establishes your place.
Kentucky waiver applications are submitted online through kynect or in person at an ADRC or Community Mental Health Center. CHFS states that applications are not accepted by mail — WaiverPath organizes your documents, dates, and follow-up calls around the kynect submission.
Texas HCS and TxHmL waiver enrollment starts through your local LIDDA — contact them first. CLASS enrollment goes through a CLASS program provider.
Ohio DODD waiver enrollment starts through your County Board of Developmental Disabilities. PASSPORT starts through your Area Agency on Aging.
Pennsylvania ODP waiver enrollment starts through your county Intellectual Disabilities/Autism (ID/A) office. Community HealthChoices (CHC) starts through your county Assistance Office or Area Agency on Aging.
