Confirm the Katie Beckett fit
Katie Beckett (TEFRA) is for children under 18 who meet an institutional level of care and can be cared for at home cost-effectively. Parent income and assets are not counted — only the child's — which is why Georgia families call it the Deeming Waiver.

Get the current packet from the official DCH page: the Katie Beckett Medicaid Application (Form 94, English or Spanish), Pediatric DMA-6(A), DMA-706 Medical Necessity/Level of Care, and DMA-704 Cost-Effectiveness. The full DCH packet with instructions runs about 41 pages.

Schedule the physician visit early
The child's physician completes the Pediatric DMA-6(A) and DMA-706. Every item on the DMA-6(A) must be answered (N/A is allowed), and the form is valid only 90 days from the physician's signature — book the appointment early and plan to mail promptly after signing.

Complete the Form 94 application
Fill out the Katie Beckett Medicaid Application (Form 94) for the child, checking the Katie Beckett block. Answer every question completely; incomplete packets slow the review and trigger missing-information requests.

Assemble medical documentation for Alliant

Submit the packet to the Norcross intake team
Mail to the Centralized Katie Beckett Medicaid Team, 2211 Beaver Ruin Road, Suite 150, Norcross, GA 30071, or fax 678-248-7459. Since April 15, 2026 you can also upload documents through the Katie Beckett portal. Keep a full copy of everything you send.

Respond within 30 days to missing-info requests
If the packet is incomplete, families get 30 days to supply the missing information to Alliant. Log the request date the day it arrives and set a reminder well before the deadline.

Note the renewal horizon after approval
Level-of-care approvals are now authorized for no less than 2 years. Record the approval date and set a renewal check about 22 months out so the next review never sneaks up.
