Choose the Virginia DD or CCC Plus pathway
Virginia groups its Medicaid HCBS pathways into three Developmental Disability waivers and the CCC Plus waiver. DD waivers serve people with developmental disabilities; CCC Plus serves older adults and people with physical disabilities, chronic illness, or substantial skilled-nursing needs. Confirm the pathway with the administering agency before starting intake.

For a DD waiver, contact the local CSB or BHA
The first DD-waiver step is an intake with the Community Services Board or Behavioral Health Authority serving the applicant's locality. Ask for a DD eligibility screening and record the date, staff member, and documents requested.

Complete the DD eligibility and needs assessment
The local board documents developmental-disability eligibility and functional support needs. Virginia uses the VIDES process for eligibility and a Supports Intensity Scale assessment for nearly everyone receiving DD waiver services. Keep copies of evaluations and every assessment notice.

Record DD waitlist priority and every annual contact
Virginia's DD waivers have a waiting list and slots are assigned by urgency of need. Ask the support coordinator to confirm the applicant's priority category, contact information, and what must be updated when circumstances change.

For CCC Plus, request an LTSS screening
Request a Long-Term Services and Supports screening through the local Department of Social Services. A community screening team assesses whether the applicant meets the level-of-care pathway; a hospital discharge planner can arrange screening for someone who is hospitalized.
