Ohio's current Form ODM 02399 says a person can submit the completed waiver-request form to the local County Department of Job and Family Services or call an Ohio Benefits Long-Term Services and Supports agency at 1-844-644-6582. The operating agency then determines the correct assessment and program path.
The current federally approved Ohio I/DD waivers are Individual Options, Level One, and Self-Empowered Life Funding. Start with the local county board of developmental disabilities and confirm current intake requirements.
Current Ohio rules require applicants to be age 59 or younger, eligible for Ohio Medicaid, complete an in-person comprehensive assessment, meet a nursing-facility level of care, and satisfy the program's other enrollment conditions. Ohio Medicaid makes the decision.
