How to request Ohio Home Care
Step 1
Check the current program basics
Ohio's current rule covers people from birth through age 59 who are eligible for Ohio Medicaid, complete an in-person comprehensive assessment, and meet a nursing-facility level of care, along with the rule's other conditions.
Step 2
Start or confirm Ohio Medicaid
The current state overview says the local County Department of Job and Family Services determines Medicaid financial eligibility. A person must be enrolled in Medicaid before waiver approval.
Step 3
Request the waiver
Submit Form ODM 02399 to the local county JFS office, or call an Ohio Benefits Long-Term Services and Supports agency at 1-844-644-6582 instead of submitting the form.
Step 4
Record the comprehensive assessment
Save the assessment date, assessor, functional and nursing needs discussed, records requested, and any priority category the agency confirms.
Step 5
Track the written decision and capacity
